Health Focus Group

Compensation below for Part 1 & Part 2

Part 1: $35 (if chosen) for full-participation
Part 2: $125 (if chosen) for full-participation

This is an ONLINE Project, you can only participate using a computer with working webcam and working mic.

Open to  ages 18-64

Part 1, $35 - if selected and fully participate
•Online Survey: Length: 30-45 minutes
•Must have high speed internet AND use a desktop/laptop computer to complete the survey.  Must have a Webcam to participate in Part 2, the follow up Webcam Group
•Cannot use a smartphone or tablet
•Use Google Chrome or Mozilla Firefox (Internet Explorer is not compatible)
•Survey must be complete between Wednesday, October 29th at 12pm ET – Sunday, November 2nd by 12pm ET.  
•Survey can be complete at any time during this time frame (please complete as soon as possible)
•Must be complete by Sunday, November 2nd before 12pm EASTERN
•Complete the entire survey at one time, do not log in and out to complete at a later time.

Part 2 - $125 if selected and fully participate
•Follow Up Webcam Group: After completion of Online Survey, Some will be selected to participate in a 90 minute Webcam Group, held Monday, November 3rd
•If selected, you will be notified by email with the details.
•Incentive: 90 minute Webcam Group: $125

•Not everyone will be needed for part 2.  Some will be released at the start of Part 2.
